I understand that the personal information collected by Southdale Community Centre will only be used for administration of programming and may be shared with authorized representatives of the Centre. My image may appear incidentally in promotion material although the Centre will do their best to obtain approval prior to publication.

I am aware that my participation in programming involves risks which could result in physical injury, illness, damage or loss. I agree to waive any and all claims by myself or next of kin for any and all liability for any of these losses that I may suffer as a result of my participation, including negligence.

In the event of an emergency, I understand that 911 may be contacted for assistance.
